How not to document – learnings for CDS, HIM/Coders & Clinicians

Glenn and Robin bring their extensive knowledge of clinical documentation, quality care and clinician engagement to look at clinical documentation practices that reflect both complexity of care and healthcare funding. Using real case studies, in this webinar they consider clinical documentation practices that may be denied by insurance providers in the private setting or in public health be picked up in the internal/external audit cycles as not supporting clinical code assignment; which in both cases can result in payment denial, and missed or inaccurate funding outcomes.

 

CDS, HIMs and Clinical Coders in both the private and public settings can apply the findings from the case studies to understand how to support documentation improvement on the ward and within the record to optimise documentation practices for funding and care.

 

This information will also be invaluable when designing and delivering CDI education that supports improved documentation across the continuum of care.  This will facilitate safe, coordinated care, reduce the burden of coding queries and ensure clinical documentation practices effectively, clearly and concisely communicate the patient care provided.
